I heard this term used from the guy (he does not leave his name) who runs the www.florisene.com website. This happens when you have TE and the hair does not grow back immediatley after shedding. The hair that falls out from the TE assault (in my case anemia and surgery)is not replaced and thus is left "empty". This explains how we see the hair loss--the "spacing" in between each follicle. My scalp has been this way since the end of 1999--this is called "lag phase" TE when the TE does not heal in the "normal" time (6--12 months). Following my 4 surgeries I lost so much hair (50%) and it was not enough to remove the trigger(drugs from surgery) to grow back the hair. Hair can't grow back until the health is restored. Ferratin 70+, folic acid levels need to be normal, B12, protein levels--this all takes time. This is why nutrition and IRON is key in recovering from TE.
